Transitional Care Management
Transitional Care Management programs target the transfer from inpatient to the community environment.
Our clinical team supports care transition, communicates with providers to schedule follow-up appointments within seven to 14 days of discharge, completing CMS requirements in obtaining the medical records from the facility of discharge.

Education
Our TCM service provides educational materials delivered straight to a patient's home to learn more about conditions and find solutions that will work best.
Communication
We provide access to medical professionals via email, phone or video chat whenever needed. Whether it's an emergency or just a general question, they'll be available to help
Follow-up
Med-Kick also offers follow-up care when needed in order for patients to comply with treatment recommendations and support long-term health.
